Sex-hormone transport (SHBG) is a genetic trait Biome reports in its hormone pathways panel: Two common variants that shift how much sex hormone-binding globulin you carry — about 11% per copy of the lead variant, roughly 23% between the two ends, and slightly stronger in men than women. SHBG ferries testosterone and estradiol through the blood, so it changes what a total testosterone reading means, not how much active hormone you have. Measured only in European-ancestry groups, and the lead variant is close to absent in East Asian groups; ask for SHBG and free testosterone alongside a total testosterone test.
